HIPAA Risk Assessments, Demystified
A HIPAA risk assessment is not a checklist you buy once; it is a living map of where your patient data lives and what could go wrong.
What it actually is
The HIPAA Security Rule requires a risk analysis. Not a policy binder, not a one-time audit, but a documented process where you find every place electronic protected health information lives, name the threats to it, and rate how likely and how damaging each one is.
Most penalties after a breach are not for the breach itself. They are for never having done a real, current risk analysis. If you cannot show your work, regulators assume the work was never done.
Start by following the data
List every system that touches PHI: your EHR, your billing platform, email, imaging, backups, the laptop a clinician takes home, the third-party scheduling app, and the cloud where it all syncs. Include the boring stuff. A fax server and a shared drive have both caused real breaches.
For each system, write down where the data sits, who can reach it, and how it leaves the building. The map is usually larger than people expect, and that gap is the point of the exercise.
Rate, prioritize, and fix
For every risk, score the likelihood and the impact. An unencrypted laptop is high on both. A locked server room with two staff is lower. This scoring is what turns a vague worry into a ranked work list you can actually fund.
Then write a remediation plan with owners and dates. You do not have to fix everything at once. You do have to show steady, documented progress on the highest risks first.
Keep it alive
